A Community-Led Model of Care and Support

 

The Agbara Programme is Sentinal Care Alliance’s core intervention model, providing structured, responsive support to individuals and families experiencing crisis, instability or barriers to accessing care.


The Agbara Programme is a culturally responsive delivery programme designed to provide holistic health, wellbeing, and social care support within community and family-based 

environments.

Delivered through Sentinal Care Alliance, Agbara offers an alternative to traditional, institution-led care by focusing on relationships, cultural identity, and community connection as key drivers of wellbeing.


What the Agbara Programme Delivers


Agbara provides flexible, person-centred support pathways that respond to individual needs while strengthening family and community networks.


 Family-Based and Nurture Placements

  • Safe, supportive home environments
  • Step-down from hospital or crisis services
  • Prevention of placement breakdown
  • Support for mental health, recovery, and stability


Community-Based Outreach

  • Engagement within trusted settings (barbers, salons, faith spaces, community hubs)
  • Peer-led support through Wellbeing Interseders
  • Early intervention and prevention-focused support

Supporting individuals and families experiencing or at risk of domestic abuse through:

  • Strengthening stability and recovery


Crisis Prevention and Alternative to Admission

Providing early intervention and community-based support to reduce the need for hospital or crisis service attendance.


Step-Down and Recovery Support

Supporting individuals transitioning from inpatient or crisis services into stable, community-based environments.


Supported Living and Nurture Pathways

Delivering structured, family-based and supported living approaches for:

  • Adults with mental health needs
