Nurture Project

  

Purpose

The purpose of Sentinal Nurture Placements is to provide a safe space in the community for Global Majority indivduals whom:

1. Young People whom have left care or been identified as having extra support needs. 

2. Individuals whom have been discharged from hospital and require rehabilitation support.

3. Individuals experiencing Mental health challenges and those displaying signs of crisis to be provided with 24 hour supervised accommodation where individuals are assessed and supported through direct work with partnering agencies such as social services, community mental health teams and GP’s. Individuals will be supported with the management of their mental and physical wellbeing as an intervention to reduce the need for hospital admission. Our community-based provisions facilitate early detection, intervention and sustained care and treatment.


Our bespoke care plans are put in place based on service users needs and wishes document in ACD (Advance Choice Document) and information collocated through our crisis referrals, allowing services users to have more control in how their lives are managed should they become unwell and unable to express their wishes in how to receive care. Individuals are encouraged and supported to work through required steps to recovery to enable a smooth reintroduction back home.


There is extensive literature on the benefits of crisis residential as an alternative to hospital admissions. Such services increase the choices for service users who are in a mental health crisis and their families, allowing greater flexibility around clinical and physical care. 

Community-based alternatives to hospital are known to provide greater service user satisfaction and are known to provide a fewer negative experience. It is hoped that the facilitation of our nurture placements will also assist in reducing the pressure on NHS Mental Health Services for individuals that are in mental health crisis who would otherwise end up being admitted to hospital

Our acute and crisis care provisions were developed with a clear vision to provide high quality, person- centred, culturally appropriate support to individual's aged 16 years +

Support Staff will observe and provide a range of non-clinical assistance to service users, whilst all clinical management and treatment of service users referred to our nurture placements will be provided by our partnering home treatment /community treatment teams as part of the individual's care. Sentinal has identified the need for nurture placements to be culturally sensitive paying close attention to all ethnicity. With our focus being on holistic wellbeing and recovery.

Service Delivery

Our Nurture placements means we work in direct partnership with multi-disciplinary teams supporting the acute and urgent care pathways, local authority and private referrals. Access to our mental health support services comes directly from referring partners NHS Trusts, GP’s and we also accept self-referrals to the Out of Hours Crisis Team if placement is agreed by home treatment  teams.

Our Nurture Placements are also a post –discharge option as way of shortening in patients stay once individual is recovered and ready to be supported back home into the community. 

Interventions at the nurture placements focus on helping individuals to build their skills, confidence and self – esteem whilst managing their mentalUand physical wellbeing. Service users are encouraged to continue with their day-to-day life.  

1. Placements will always house no more than 2 service users at a time with ratio of 2 engagement staff allocated to each placement (unless risk assessed to need more).

2.  There will always been 24-hour support from on-call care supervisor in event of emergencies.

3.  On referral each service user is allocated a engagement officer who will be directly responsible for all observations and correspondence with colleagues and support systems for service user.

All staff will be trained in mental health awareness, mental health first aid and crisis support work. Service users' observations and care plans will be stored and reviewed via our CRM system.  

Advance Choice Documents are mandatory for all of our service users and is completed during transition into placement. Information is used to create a care plans that are co-produced with the service user and Sentinal and will set out the actions required to identify and support' well-being, care and support needs. This plan will be reviewed .
